You can ask for a copy of a Felix prescription for your records, for example to share with another doctor or your insurance company. There's no charge for this.
By law, you're entitled to a copy of your prescription. The copy is marked VOID, so it can't be used to fill the prescription at a pharmacy. Your prescription itself stays active.
How to request a copy
Download the form: Prescription Void Request form (form can be found at the bottom of this article).
Fill it in on your phone or computer. It's a fillable PDF: tap or click each field and type your answers. Fields marked with * are required. You'll need:
Your name, date of birth, phone number, email, and address
The name of the medication
The date the prescription was issued (see the tip below)
Who should receive the copy. If anyone other than you should receive it, add their name and fax number.
Sign it. Type your full name in the signature field and add the date. Your typed name counts as your signature.
Save the completed form.
Send it to the Felix support team. In the Felix app, go to the Chat tab, tap View support messages, open your conversation with the support team, and attach the completed form there. If you don't have a support conversation yet, start one with Ask Felix to connect with support.
Not sure when your prescription was issued? Open your chat with your Felix practitioner and find the message saying your prescription was sent to the Felix pharmacy. Use the date of that message.
Need more than one prescription?
This form covers one prescription. If you need copies of several prescriptions, or prescriptions from different health categories, use the Release of Records Request instead.
Want to fill your prescription at another pharmacy?

How long it takes
Most requests are processed within 3 to 5 business days, and never more than 30 days.
