You can ask Felix to send a copy of your health records to yourself, another healthcare provider, your insurance company, or someone else you choose. There's no charge for this.
What you can request
Your Felix records can include:
Your assessments
Your prescription history
Your lab results
Your pharmacy records, including dispensing records and any notes from a Felix Pharmacy pharmacist
You can ask for all of your records, or only part of them. For example, only your lab results, prescriptions from a certain date range, or everything for one health category, such as Weight Loss or Hair Loss.
How to request your records
Download the form: Release of Records Request form (form can be found at the bottom of this article).
Fill it in on your phone or computer. It's a fillable PDF: tap or click each field and type your answers. Fields marked with * are required. You'll need:
Your name, date of birth, phone number, email, and address
Who should receive the records. If anyone other than you should receive them, add their name and fax number. Felix needs a fax number to send records to someone else securely.
Which records you want
Sign it. Type your full name in the signature field and add the date. Your typed name counts as your signature.
Save the completed form.
Send it to the Felix support team. In the Felix app, go to the Chat tab, tap View support messages, open your conversation with the support team, and attach the completed form there. If you don't have a support conversation yet, start one with Ask Felix to connect with support.
Tip: fill in every section. Incomplete forms are the most common reason requests take longer.
How long it takes
Most requests are processed within 3 to 5 business days. Larger requests can take longer, but never more than 30 days.
Records are sent securely, by encrypted file transfer or fax.
Good to know
Your consent lasts three months from the date you sign. You can withdraw it at any time before your records are sent.
Requesting for someone else? If you're a substitute decision maker (you have legal authority to act for the patient), select "I am the Substitute Decision Maker" on the form and explain your relationship and authority.
Only need a copy of one prescription? See How to get a copy of your prescription.
