Insurance Special Authorization Forms: Can my Felix doctor or nurse fill out additional forms from my insurance provider?

Insurance providers sometimes require additional forms (special authorization forms) to be completed in order to approve coverage.

If your insurance provider requires additional information, such as a special authorization form, before approving coverage for your medication, Felix can assist with the process.


Important items to note:

  1. A visit does have to be submitted before we can send your insurance forms to a healthcare practitioner to sign, as you need to be their patient.

  2. The completion of insurance or medical forms is at the discretion of your prescribing healthcare practitioner.

  3. There is a $35.00 fee per insurance form.

  4. Completion of the form does not guarantee approval by your insurance provider.

  5. It’s important that you carefully review all eligibility criteria listed on your form to confirm your qualification.

*Your healthcare practitioner will complete the insurance form using only healthcare information available in Felix’s records, and it must pertain to treatment provided at Felix.


Ensure that:

  1. All primary patient information is filled out, signed, and dated by you!

  2. Practitioner / medical sections are left blank


Next steps once your form is submitted:

  1. We will forward them directly to your practitioner to further review and complete.

  2. Once the form is completed, this will be emailed to you directly.

  3. After receiving your completed insurance forms, you will need to submit them to your insurance provider for further review.

*Due to high volume, the current turnaround time for form completion is about 3-5 business days.
