A medical note (or medical form) is written documentation made by a licensed healthcare provider to record a patient's health status, care, treatment and/or medical encounter.
How to Request a Medical Note:
You can request a medical note from your Healthcare Practitioner. Be sure to specify that you require a medical note and consent to the $25 fee. Our team will be happy to help and will respond as soon as possible.
Key considerations:
Medical notes are approved at the sole discretion of your healthcare practitioner.
In rare cases where your illness relates to the health category you're being treated for at Felix, your practitioner may be able to complete sick time forms. General sick notes are not typically provided.
If your request is approved, a $25 fee applies upon completion of the note. If it's declined, you won't be charged.
Due to high volume, the current turnaround time for medical notes is 3–5 business days.
